The short answer
Add the questions to the event: a yes or no to "Do you have any of the following conditions", a free-text emergency contact, whatever your practice requires. Mark them required, and a ticket cannot be bought without an answer.
The answers sit against the order, on Orders and in the attendee export, so the facilitator reads them the afternoon before rather than from a pile of forms at the door.
Where your insurer wants a signature, put the waiver where people can read it before they book and say in the description that booking is acceptance. That is a policy decision, not a feature, and it is yours.

A checkout question is a record of an answer. A waiver that needs a signature is a document, and this page does not pretend otherwise.

One Ticket for a recurring room
Everything below follows from one decision: a series is one object, not a list of dates.
